Your First Appointment

For your first appointment please print out the forms below and bring it to the office with you along with a copy of your health insurance card and a valid picture ID such a drivers license or passport.

Missed Appointments

Unless canceled, at least 24 hours in advance, our policy is to charge for missed appointments at the rate of $25.00 per missed appointment. Please help us serve you better by keeping scheduled appointments.
